The high rate of drug replacement restricts the recovery of population size and genetic diversity in evolutionarily rescued populations, reducing the probability of future evolutionary rescue events should the environment change. Through experimentation with Pseudomonas fluorescens and the dual antibiotics chloramphenicol and rifampin, we verified this hypothesis. The enhanced frequency of drug rotation suppressed the possibility of evolutionary rescue, leading to a considerable proportion of surviving bacterial populations exhibiting resistance to both medications. Drug treatment histories exhibited no disparity in the significant fitness costs incurred due to drug resistance. A correlation existed between population sizes at the commencement of drug treatment and the ultimate destinies of the populations (extinction or persistence), indicating that population size rebound and adaptive evolution in advance of the drug transition elevate the probability of population survival. This research aimed to explore the persistence of antithrombotic medication use in the peri-colorectal endoscopic submucosal dissection (ESD) procedure.
Forty-six-eight patients with colorectal epithelial neoplasms, undergoing ESD treatment, were included in the study. Among these, 82 were taking antithrombotic medications and 386 were not. Patients receiving antithrombotic medications persisted with these agents throughout the peri-ESD period. Following propensity score matching, clinical characteristics and adverse events were compared.
Patients continuing antithrombotic medications experienced a higher post-colorectal ESD bleeding rate, both before and after propensity score matching, compared to those not taking such medications. Specifically, the bleeding rate was 195% and 216%, respectively, for the former group, and 29% and 54%, respectively, for the latter group. Continued use of antithrombotic medication was shown in Cox regression analysis to be associated with a substantially increased risk of post-ESD bleeding, with a hazard ratio of 373 (95% confidence interval: 12-116), and a statistically significant association (p<0.005) when compared to patients without antithrombotic therapy.
