In the general population of postmenopausal women, obesity, as indicated by a body mass index (BMI) of 30, has been established as a risk element for breast cancer. The unclear nature of elevated BMI as a risk factor for cancer in women with BRCA1 or BRCA2 germline mutations is a consequence of both the inconsistent outcomes of epidemiological investigations and the paucity of mechanistic studies targeting this specific population. This study demonstrates a positive association between BMI, metabolic dysfunction markers, and DNA damage in normal breast epithelia of women with a BRCA mutation. Obesity-related modifications of the breast adipose microenvironment, as demonstrated by RNA sequencing, were observed in BRCA mutation carriers, specifically including the activation of estrogen biosynthesis, leading to impacts on neighboring breast epithelial cells. This study investigated the effectiveness of functional testing relative to invasive coronary angiography (ICA) for acute chest pain patients who initially underwent coronary computed tomography angiography (CCTA) and exhibited intermediate coronary stenosis, defined as 50% to 70% luminal narrowing.
Our retrospective analysis included 4763 acute chest pain patients, aged 18 years or above, whose initial diagnostic approach was a CCTA. In the patient cohort, 118 satisfied the enrollment criteria, with 80 progressing to stress testing and the remaining 38 proceeding straight to ICA. The primary endpoint was a 30-day major adverse cardiac event, including acute myocardial infarction, emergent revascularization, or fatality.
Initial stress testing versus direct referral to interventional cardiology (ICA) post-coronary computed tomography angiography (CCTA) demonstrated no difference in the incidence of 30-day major adverse cardiac events.
